- EVV
Sinq provides electronic access to schedules and allows care givers to document time and location of visits real time. Because Sinq uses a native technology, the application functions whether the user has data access or not at the time of the visit. Sinq collects time and location stamps only explicitly upon Check In and Check Out. No location or time data is polled by the Application and Sinq has no passive access to user information.
- Care Plan
Sinq provides prescriptive guidance of defined tasks at the individual level to ensure "compliance by design". No more paper forms or guesswork to determine what tasks are to be performed. The application provides guidance before, during and after visits to ensure that care givers are able to quickly and accurately reflect a job well done.
- Care Gaps
Sinq works with partnering Health Plan organizations and claims data to identify, qualify, and prioritize potential gaps in care. Gaps are identified based on a person's health history and demographics, and are shared with Sinq's partnering Provider organizations. Care Givers receive visibility to Gaps related to the people they care for and are able to collaborate with both Plan and Provider teams real time.
- Changes in Condition
Sinq uses curated obervational guided enquiry to diagnose, identify, document and escalate potential decompensations before they result in adverse events. Avoiding more serious problems is what this is all about.
